MRI of the liver finds and describes liver lumps, fatty liver and iron overload without radiation. Learn when it is advised and how to prepare.
An MRI liver scan gives detailed images of the liver and can tell whether a lump is a harmless cyst or hemangioma, or something that needs treatment. Special sequences can also measure the amount of fat and iron in the liver. It uses no radiation and is often done with contrast.
What Makes MRI Special for the Liver?
MRI separates tissues by water, fat and blood content, so it is excellent at telling liver lesions apart. Liver-specific contrast agents can improve detection of small deposits. Fat and iron mapping give numbers without a needle biopsy. See MRI abdomen scan.

- A liver spot found on ultrasound or CT that is unclear
- Hepatitis B or C or cirrhosis with a raised AFP or new lesion
- Before liver surgery or ablation
- Known cancer elsewhere, to look for liver spread
- Fatty liver or iron overload measurement in selected cases
Preparation and Procedure
Fast for 4 to 6 hours. Practise breath-holding for 15 to 20 seconds, as many images need a still breath. Remove metal. Contrast is injected during the scan, with kidney checks. The scan takes 30 to 45 minutes. See MRI preparation and MRI with contrast.
MRI vs CT of the Liver
CT triple phase is quick and widely available. MRI usually characterises small lesions better and avoids radiation and iodine. Many patients with cirrhosis alternate or combine them. Read CT liver triple phase scan.
Understanding Fatty Liver Results
Fatty liver is very common. MRI-PDFF gives a percentage of fat in the liver, and elastography on MRI or ultrasound estimates stiffness (scarring). Weight loss, exercise, less sugar and refined carbohydrate, controlled diabetes and limiting alcohol can reduce liver fat and improve health.
A Practical Example
Rina, 48, has a 3 cm liver spot on CT that could be a hemangioma or something else. An MRI shows the classic bright, slowly filling pattern of a hemangioma, so no treatment is needed. In another patient with cirrhosis, an MRI shows a nodule with cancer features, leading to early treatment.
Care Tips and Key Facts
- Fast and practise breath-holds
- Reduce alcohol and sugar if the liver is fatty
- Get hepatitis B vaccination if you are not immune
- Follow screening if you have cirrhosis
After the Scan and Next Steps
Take the MRI to a hepatologist or gastroenterologist. Benign lesions often need only reassurance. Suspicious lesions may need biopsy, ablation, surgery or other treatment. Keep your weight and sugar under control, avoid alcohol if the liver is damaged, and follow your regular screening schedule.
Questions to Ask Your Doctor
- Is my liver lesion harmless or does it need treatment?
- Do I need regular scans?
- What is my liver fat or stiffness?
- What lifestyle plan will help my liver most?
Common Mistakes to Avoid
- Continuing alcohol with liver damage.
- Eating before the scan.
- Skipping follow-up of a liver nodule.
Booking, Timing and Report
Book a slot of about 45 to 60 minutes, and arrive 15 minutes early for the safety form. Carry your doctor's prescription, implant cards and old scans. Reports are usually ready within 24 to 48 hours, and you can request images on a CD or digital link for your specialist.
Understanding Your Liver Report
The report may use LI-RADS grades, mention fat percentage and describe signs of cirrhosis such as an irregular edge and enlarged spleen. Small cysts and hemangiomas are very common and harmless. Ask your doctor which findings matter for you.
Frequently Asked Questions
What can MRI liver show?
Lesions, fatty liver, iron and signs of cirrhosis.
Do I need contrast?
Often.
Do I need fasting?
Yes, 4 to 6 hours.
Is MRI better than ultrasound for the liver?
It is more detailed; ultrasound is first-line.
Can MRI measure liver fat?
Yes, with special sequences.
Is it safe in pregnancy?
Without contrast when essential.
How long does it take?
About 30 to 45 minutes.
Can MRI diagnose liver cancer?
It can strongly suggest it in at-risk patients.
When is the report ready?
Usually 24 to 48 hours.
