MRI of the neck spine shows discs, nerves and the spinal cord. Learn when it is needed for neck pain, arm numbness or weakness and how to prepare.
An MRI cervical spine scan shows the bones, discs, nerves and spinal cord of the neck. It is advised for neck pain with arm pain, tingling or weakness, after injury, or when there are signs of spinal cord pressure. Most simple neck pain improves without any scan.
What Does MRI Show in the Neck?
The neck spine has seven bones (C1 to C7) with discs between them. MRI shows these discs, the spinal cord, the exits of nerves, the ligaments and the soft tissue clearly, without radiation. X-rays show only bones, and CT is better for fractures. See MRI spine scan for the general picture.

- Neck pain with arm pain, numbness or weakness lasting more than 4 to 6 weeks
- Weak grip, dropping things or difficulty walking
- Neck pain after a fall or road accident
- Fever, weight loss or cancer history with new neck pain
- Before neck surgery or injection
Most neck pain from posture or muscle strain does not need MRI.
Common Findings
Reports often mention disc bulges, bone spurs (osteophytes), narrowed canal and nerve exit narrowing. These are common after age 40, and many people have them without pain. The finding matters when it matches your symptoms. See how to read your MRI report.
Red Flag Symptoms
Go to a hospital urgently for weakness in both arms or legs, loss of bladder or bowel control, sudden numbness, or trouble walking, since these may signal spinal cord pressure. Early treatment can prevent permanent damage.
Preparation and Procedure
No fasting is needed. Remove metal and tell the staff about implants. You lie on your back with a coil around your neck, and the scan takes 20 to 30 minutes. Keep your head still and swallow gently. Contrast is rarely needed. See MRI preparation and MRI procedure.
A Practical Example
Rohit, 44, works long hours at a computer and has neck pain with tingling in the right thumb for two months. Physiotherapy gives little relief. His MRI shows a C5-C6 disc bulge pressing on a nerve root. His doctor advises posture changes, neck strengthening and a short course of medicines, and his tingling settles in weeks.
After the Scan and Next Steps
Show the MRI to an orthopaedic surgeon, neurologist or spine specialist. Treatment usually starts with physiotherapy, posture correction, short-term pain relief and activity changes. Surgery is kept for weakness, cord pressure or severe pain that does not improve. Adjust your desk and screen to eye level, take breaks every hour and avoid carrying heavy loads on one shoulder.
Questions to Ask Your Doctor
- Which nerve or level is causing my symptoms?
- Do I need physiotherapy, an injection or surgery?
- Is there any pressure on my spinal cord?
- Which activities should I avoid?
Common Mistakes to Avoid
- Moving the neck during the scan.
- Using neck manipulation before the MRI result is known.
- Ignoring weakness or walking difficulty.
Booking, Timing and Report
Book a slot of about 45 to 60 minutes, and arrive 15 minutes early for the safety form. Carry your doctor's prescription, implant cards and old scans. Reports are usually ready within 24 to 48 hours, and you can request images on a CD or digital link for your specialist.
