What is follicular study?

Follicular study or follicular scan uses ultrasound usually a TVS (transvaginal) scan for studying and monitoring the follicles inside your ovary. It helps in counting the number of follicles, monitoring their growth and development, planning infertility treatments and guiding procedures such as IUI (Intrauterine insemination) and IVF (in-vitro fertilization). 

Why do I need follicular scan?

Follicular scan or follicular monitoring ultrasound is indicated in following situations:

  • Infertility
  • PCOS patients who want to conceive.
  • Premature ovarian failure.
  • To evaluate ovulation related abnormalities.
  • To monitor follicular size following ovulation inducing drugs such as clomiphene citrate.  
  • To plan the time of ovulation triggering agents such as hCG injection.
  • To monitor the changes in the endometrium (uterine lining) for successful implantation.
  • To guide procedures such as IUI (Intrauterine insemination), egg retrieval and IVF (Invitro fertilization).
  • To monitor the effectiveness of drugs used for the treatment of infertility.
  • To adjust the doses of fertility drugs and to monitor their side effects such as OHSS (Ovarian hyperstimulation syndrome).

When should follicular monitoring be done?

Follicular study is started from Day 9 or Day 10 of menstrual cycle and is continued till the follicle attains maximum size and ruptures to release the Secondary oocyte (Ovulation).

How many times follicular study is done?

Usually, 5-6 scans may be done in one menstrual cycle. A daily follicular monitoring or monitoring on alternate days may be performed depending upon your gynecologist’s order.

What is a good follicle count for pregnancy?

6-10 antral follicles are usually considered good or adequate for conceiving and getting pregnant. <6 antral follicles indicate a low ovarian reserve with less probability of successful ovulation, fertilization and pregnancy.

What is the size of follicle required for pregnancy?

Follicular size varies throughout the follicular phase under the influence of hormones such as FSH, LH and Estrogens. Usually when the follicles attain a size of 18-20 mm in diameter then

ovulation occurs either naturally or with the help of ovulation triggering agents. However, for pregnancy to happen, the prerequisites are fertilization and successful implantation.

How does follicular study help in conceiving?

Follicular study monitors the growth of follicle. When the follicles attain appropriate size then ovulation triggering drugs are given and patient is counselled to have intercourse or artificial insemination for a successful fertilization and implantation. This way it helps in conceiving easier to those who are unable to conceive due to various reasons.

Can follicular study confirm pregnancy?

Follicular study is used for follicular studying and monitoring purpose. It is not used to confirm pregnancy. However, to confirm pregnancy, urine pregnancy tests (UPT) or obstetric ultrasound are usually done.

What injection is given after follicular study?

Your gynecologist will administer an injection of hCG (human chorionic gonadotropin) usually when the follicle inside your ovary attains a size of 18-20 mm. hCG is an analogue of LH (Luteinizing hormone). It usually triggers ovulation approximately 36 hours post Injection.

After follicular study what are the next treatments taken?

After follicular study following treatments or procedures can be performed according to your condition:

  1. Injection of hCG to trigger ovulation
  2. Intrauterine insemination (IUI)
  3. Intracervical insemination
  4. Intrafallopian tube insemination
  5. Egg retrieval and IVF (In-vitro fertilization)

After follicular study when to take pregnancy test?

After follicular study, injection of ovulation triggering agent is given which is followed by planned intercourse or IUI (Intrauterine insemination) for fertilization to happen. Usually, it takes 7-10 days for implantation of embryo in the uterus. So, you should take pregnancy test at least 14 days later post follicular study or when you miss your menstrual period.

What are the side effects of follicular scan?

Follicular scan is a type of ultrasound imaging and uses sound waves to create images for follicular monitoring. It doesn’t utilize ionising radiations which are used in other imaging techniques such as X-rays or CT scan. It is a fast, non-invasive, painless and safe procedure with no risks or side effects. However, follicular scan is performed via TVS(Transvaginal) scan which requires positioning of transducer or probe inside the vagina. This may cause a little discomfort to some, especially those females who have never given birth to a baby.

What is the success rate of pregnancy after follicular study?

Studies suggest only 10 % success rate of follicular monitoring. However, success rate of follicular study and subsequent pregnancy vary from patient to patient and depends upon many variables such as:

  • Hormone levels
  • Age of patient
  • PCOS
  • Proper Infertility treatments
  • Any pelvic abnormalities such PID (Pelvic inflammatory disorders), Chronic pelvic infections, pelvic tumours, uterine anomalies
  •  Nutritional status of female
  • Chronic illness etc.
