What is Sialography?

Sialography is the imaging of salivary glands, most commonly of the parotid gland. The salivary ducts are also conventionally examined by fluoroscopy with high sensitivity, and though cross-sectional imaging with either CT or MR Sialography.

Technique of MR Sialography

MRI Sialography is known to be a fairly sensitive and a reliable method of further evaluating the salivary glands.

There is a fast acquisition of the heavily T2 weighted sequences that are (e.g. RARE, CISS, FISP, are commonly used in MRCP and MR urography).

This is used to brighten the intraluminal fluid and the display ductal morphology adequately with no further need to inject any of the contrasts into the following ducts.

The MR contrast administered intravenously is considered to be a very useful adjunct.

Before the Test

A contrast dye will be injected before the scan.

During the Test

A sialogram will be used and performed to diagnose any blockage of the salivary flow due to any stones or strictures.

This examination is done by then the introduction of a very thin tube into opening of the duct

Then inject a small amount of liquid. This probe procedure would do without any anesthesia requirement as it is considered to be very painful.

After the Test

  1. After the scanning is complete, the respective radiologist would examine the images to understand whether any more of them are required ahead. Once the radiologist is satisfied, the patient could go back home.
  2. The dye will be flushed out from your mouth. You should massage your salivary glands for ease.

The examination of your salivary glands by Sialography would reveal the detailed morphology of the salivary gland ductal system.

Most of the parotid tumors are benign in nature (not cancerous). 

The most common type, also called as pleomorphic adenoma, is a slow-growing mass that has low chance of turning malignant.

The swelling and pain in the salivary glands could be caused by blockage from either a calculus or a stone in the patient’s salivary duct. 

A Sialogram is usually performed to locate any blockage and also to diagnose what would be further causing it.

The Sialography is usually contraindicated in acute infections because of the disrupted ductal epithelium. This might allow the extravasations’ of the contrast agent, which results in foreign body reaction and causes severe pain.

However, this technique is only considered appropriate for its use in cases of any suspected chronic infections.

The scan is said to usually take anywhere between 15 to say 90 minutes.

Including the scan as well, the total examination time that is usually taken is between 1.5 to 3 hours. The substance which is called gadolinium is then injected into the vein to aid the physicians to view an image more clearly.
