What is Sialography?

Sialography is the imaging of salivary glands, most commonly of the parotid gland. The salivary ducts are also conventionally examined by fluoroscopy with high sensitivity, and though cross-sectional imaging with either CT or MR Sialography.

Technique of MR Sialography

MRI Sialography is known to be a fairly sensitive and a reliable method of further evaluating the salivary glands.

There is a fast acquisition of the heavily T2 weighted sequences that are (e.g. RARE, CISS, FISP, are commonly used in MRCP and MR urography).

This is used to brighten the intraluminal fluid and the display ductal morphology adequately with no further need to inject any of the contrasts into the following ducts.

The MR contrast administered intravenously is considered to be a very useful adjunct.

Advantages of MR Sialography

  • Rapid Acquisition
  • Non-Invasive
  • Assessment Of Other Glands Possible
  • Excellent Delineation Of Parenchymal Pathology
  • No Special Positioning Required
  • As no cannulation would be required, further risk of air bubbles simulating in the intraductal calculi is quite minimized.

Disadvantages of MR Sialography

  • General MRI Contraindications, for example-. Pacemakers, or Implants, or patients with Claustrophobia
  • Some Dental Fillings, or Implants, or Bridges, can also cause further Image Impairment
  • Only the First- And the Second-Order Branches can be delineated by this technique

Why is MR Sialography Done?

  • Repeated Pain And Swelling Of Salivary Gland
  • Suspected Sialolithiasis Or The Salivary Duct Obstruction
  • Suspected Sialectasis (Dilatation of Salivary Gland) in Chronic Inflammatory Disorders 0r Autoimmune Diseases (example- Sicca Syndrome 1)

What are the Contraindications of Sialography?

  • Pregnancy
  • Allergy To An Iodinated Contrast Media
  • Acute Sialadenitis (inflammation of salivary ducts)

Guidelines To Follow Before, During and After the Test

Here are some guidelines an individual can refer to once they have decided to undergo a MRI Sialography

The required documents needed for this test includes your valid government ID and an appropriate Doctor’s prescription.

Before the Test

A contrast dye will be injected before the scan.

During the Test

A sialogram will be used and performed to diagnose any blockage of the salivary flow due to any stones or strictures.

This examination is done by then the introduction of a very thin tube into opening of the duct

Then inject a small amount of liquid. This probe procedure would do without any anesthesia requirement as it is considered to be very painful.

After the Test

  1. After the scanning is complete, the respective radiologist would examine the images to understand whether any more of them are required ahead. Once the radiologist is satisfied, the patient could go back home.
  2. The dye will be flushed out from your mouth. You should massage your salivary glands for ease.
  3. Also, patients are required to take appointment with their respective doctor to discuss the results for future treatment planning.

FAQ

The examination of your salivary glands by Sialography would reveal the detailed morphology of the salivary gland ductal system.

The healthcare professional would perform a few physical exams and then they might order the following tests:

  • Ultrasounds
  • CT scans
  • MRI scans

This is done to further help and guide in the treatment.

Sialendoscopy might also be used to assess the stone(s).

The stones are further categorized by size of the diameter i.e. small: 4 millimeters or smaller.

The IV contrast media is either the gadolinium for MRI or an iodinated contrast for CT.

A contrast dye that has iodine as a component would be injected into your salivary duct through the tube.

The dye then blocks the rays and would produce a clear image of the glands. This would aid the doctor identify the problem ahead.

Most of the parotid tumors are benign in nature (not cancerous).

The most common type, also called as pleomorphic adenoma, is a slow-growing mass that has low chance of turning malignant.

The swelling and pain in the salivary glands could be caused by blockage from either a calculus or a stone in the patient’s salivary duct.

A Sialogram is usually performed to locate any blockage and also to diagnose what would be further causing it.

The Sialography is usually contraindicated in acute infections because of the disrupted ductal epithelium. This might allow the extravasations’ of the contrast agent, which results in foreign body reaction and causes severe pain.

However, this technique is only considered appropriate for its use in cases of any suspected chronic infections.

The scan is said to usually take anywhere between 15 to say 90 minutes.

Including the scan as well, the total examination time that is usually taken is between 1.5 to 3 hours. The substance which is called gadolinium is then injected into the vein to aid the physicians to view an image more clearly.
