What is an Inhibin-B Test?

Inhibin-B is known to be a hormone that is secreted by the specific ovarian cells. These hormones can also sometimes indicate either an Ovarian Cancer or an underdeveloped fetus.

What is the Function of Inhibin-B in the body?

This hormone regulates the menstrual cycles and allows the fetus to develop during the pregnancy period.

The Inhibin-B Blood Test, on the other hand, is generally used to determine the levels of this hormone in the bloodstream.

It aids in determining a woman's ovary (reserve) as a measure of the chances of her becoming pregnant. If the woman is already pregnant, it would help in the detection of pregnancy anomalies such as - Ovarian Cancer.

Why is Inhibin-B Test done?

This test is used to assess the function of the antral follicles of the ovaries in women or the Sertoli cells of testes in men.

 It serves as adjunct to follicle-stimulating hormone testing during the infertility evaluation.

Inhibin B is made mostly by ovaries in females, and by testicles in males.

  • In women Inhibin B is generally associated with the maturation of follicles in ovaries. The follicles are said to be those structures that develop into the mature eggs. The Inhibin B rise and fall at different periods/times during each of the menstrual cycle.
  • In men: Inhibin B plays an important role in production of the sperm in testicles. Inhibin B levels are said to be higher in men with a normal fertility compared to those men who would not produce an adequate number of the viable sperms.

These roles allow the Inhibin- B tests to be used as a marker of the remaining fertility and the ovarian function in the women.

Note: This test is less commonly used to assess the fertility in men.

Guidelines to follow before, during and after the test

Before the Test

  1. No specific preparation is required before the test.
  2. NO fasting is necessary before undertaking the test.
  3. Specimen preparation: Moreover, the area would be cleaned very carefully before the blood is drawn from the vein.

During the Test

Method: When blood is drawn, it is called “venipuncture

  1. At the test center, you would be requested to sit comfortably on the chair or even lie down on the cot.
  2. In case of long sleeve clothing, roll up sleeve of one end or remove the arm from the sleeve of the clothing.
  3. A healthcare provider, or a technician/nurse, would tie band made of rubber tightly on your upper arm.
  4. Once an appropriate vein is found, a needle is inserted below the skin to take the sample.

A short sharp prick may be felt when needle puncture is made under the skin, to collect the blood sample.

After the Test

  1. After the blood is withdrawn, the respective technician/ healthcare professional would withdraw the needle.
  2. Place pressure on puncture wound to stop the bleeding.
  3. You would be able to return on your usual routine immediately.

The Inhibin-B controls the FSH secretion via a negative feedback mechanism. It is a glycoprotein hormone which is secreted by Sertoli cells of testis and granulosa and theca cells of ovary. 

The Serum Inhibin-B concentrations are said to be positively related to the testicular volume and sperm count.

There is no specific preparation is required for this particular test.

The Normal range for the Inhibin-B is said to be: 10–200 pg/mL. The Normal range for the follicular phase estradiol level is said to be: 69 to 905 pmoL/L.

The Elevated level of the Inhibin-A and/or the Inhibin-B may be present in the blood of woman with the rare type of ovarian tumor. This is called granulosa cell tumor.

The Blood levels of these hormones might also be elevated in some of the women that have mucinous epithelial tumors.

The studies in women have also demonstrated that a decreased Inhibin-B secretion is generally associated with an elevated FSH levels and a decreased oocyte quality and the fertility potential.

 Hence, Inhibin-B is used as a predictor of the ovarian reserve.

The FSH stimulates secretion of Inhibin from granulosa cells of ovarian follicles in the ovaries. In turn, the Inhibin suppresses FSH.

The Inhibin-B reaches a peak, say in the early- to mid-follicular phase, and a second peak is seen at ovulation. The Inhibin A reaches at its peak in the mid-luteal phase.

The Hormone levels such as the Inhibin-B could be further requested if the woman's ovaries appear to not be working well. 

This is generally because she might be experiencing troubles in pregnancy. More so if the woman has symptoms and signs of the polycystic ovarian syndrome, however, many-a times this test has been superseded (outdated) by anti-Mullerian Hormone (AMH).

The AMH is usually produced solely in growing ovarian follicles, the serum levels here, are regarded as the marker for an ovarian reserve, which represents the quantity of ovarian follicle pool. Inhibin-B is produced by granulosa and the theca cells and its activities include- suppression of FSH production.

In older women, in the early stages of menopausal transition, the Inhibin-B concentration is said to be significantly lowered in follicular phase but it remains unchanged or even elevated levels of E2 and Inhibin A in both follicular and luteal phase, can be seen.

The administration of FSH increases secretion of the Inhibin-B in normal men, but is seen to be much more pronounced in the males that are with secondary hypogonadism.

The Inhibin-B levels are usually detectable in cord blood of the male newborns, rise to the levels that are higher than adult range by 6 months of age, and then it begins to decline to low levels at even 3–6 years.
