What is Cryoglobulins Panel test?

Cryoglobulins are circulating proteins, specifically immunoglobulins (IgG, IgM, IgA, or light chains) that clump together (precipitate) when exposed to cold temperatures and dissolve when warmed. They may be present in trace amounts in the blood of some healthy people, but they are most frequently linked to abnormal protein production and a variety of diseases and conditions. This test detects and quantifies the amount of cryoglobulins in the blood.

Why is it done?

To determine whether symptoms such as cold sensitivity in the extremities are caused by the presence of abnormal proteins called cryoglobulins in the blood, which can be linked to a variety of diseases.

The Cryoglobulin Panel is a common and a simple clinical immunology laboratory test that usually would require the blood samples to be transported to the laboratory, without cooling, at a temperature of 37°C.

To achieve this, the samples have been generally delivered by hand to the laboratory in an insulated container containing warm water.

The Cryoglobulinemia is mostly associated with many diseases such as the following:

  • Hepatitis C
  • HIV
  • Multiple Myeloma
  • Waldenstrom Macroglobulinemia,
  • Lupus
  • Sjogren's Syndrome
  • Lymphoma

Usually, no test preparation would be needed for this test. There could be an 8-hour fast instructed before the sample collection and to minimize potential for the turbidity (or cloudiness) in sample due to the triglycerides.

The Cryoglobulins are considered to be the proteins (immunoglobulins) which would precipitate out of the serum at the temperatures that are lower than the normal body temperature. 

Although it might be noted that some cryoglobulins also precipitate at temperatures that are as high as say 30-35°C.  In the laboratory, the samples are generally cooled to 4°C to test for the cryoglobulins.

This test is usually done to find out if an individual has any abnormal proteins called the cryoglobulins in the blood. 

The Blood proteins also include the normal immunoglobulins (antibodies), for example-IgG and IgM. But they could also include the antibodies linked to the autoimmune diseases.

The Immunosuppressive drugs are mainstay of the treatment  of this disease and for the severe disease where the vital organs would be affected. 

The Corticosteroids such as the prednisone, and the immunosuppressant, for example- azathioprine or cyclophosphamide, that are widely used. 

The drug rituximab is common treatment option for the condition.

High levels of the cryoglobulins are indicative of abnormal proteins. This condition is also seen with number of different autoimmune disorders and conditions.

The ESR could be markedly elevated in the patients with Type I Cryoglobulinemia, reflecting any underlying monoclonal Immunoglobulin.

The treatment of the cryoglobulinemia neuropathy can be associated with hepatitis B which includes the nucleoside analogs, which in general suppresses the viral replication in 90% cases

The Treatment modalities include the following:

  • The Plasma exchange
  • High-dose corticosteroids, Rituximab that will be reserved for any  severe and the refractory cases.

Cryoglobulinemic vasculitis could result in any debilitating symptoms. For example- painful neuropathy or any complications, for example- gangrene that may require amputations

Its association is also with an increase in the mortality, as multiple relapses have been seen.

The False-positive results are seen to appear in patients with heparin therapy, 

The reason behind this a complex formation of heparin and the Fibronectin (in between them). 

As Fibronectin is considered to be an insoluble cold protein.
