What Is Level 2 Ultrasound in Pregnancy?

Level 2 ultrasound also known as Anomaly scan or 20-week ultrasound or 2nd trimester ultrasound is a type of prenatal scan that is used for the assessment and evaluation of anatomical details of the developing fetus, placenta as well as maternal pelvic or internal reproductive organs.

The main aim of anomaly/targeted scan is to detect congenital malformations and fetal growth abnormalities. It also identifies placental pathologies, helps in planning the mode and time of delivery and evaluates if there is any need to terminate pregnancy.

Level 2 scan is usually done between 18-22 weeks of gestation (2nd trimester). It utilizes sound waves and a computer to produce images delineating anatomical details of fetus and maternal pelvic organs. Level 2 scan relies on identification of soft markers along with pre-defined structural malformations for screening various aneuploidies such as Down syndrome.

Level 2 sonogram/scan is indicated in every pregnant woman and in each pregnancy. Usually, 2-D anomaly scan is conducted. However, 3D scans, Tvs scan and color doppler scan al though not used routinely, may be indicated for further evaluation in certain cases such as high suspicion of fetal anomalies or cases of placenta previa.

What Are the Uses of Level 2 Scan in Pregnancy?

Your obstetrician may order a level 2 scan for screening, detection, diagnosis, management and monitoring of various conditions affecting fetus, placenta or maternal pelvic organs. It can demonstrate the following findings:

    1. Chromosomal aneuploidies including Down syndrome (Trisomy 21), Patau syndrome (Trisomy 13), Edward syndrome (Trisomy 18).

      • Unicornuate uterus

How Do I Prepare for Level 2 Scan?

Follow these simple steps if you are going for level 2 sonography

  1. Take an appointment- Fix an appointment in a diagnostic center having the facility of Level 2 scan.
  2. Clothing- Dress yourself in loose and comfortable outfits so that your body parts which need to be scanned can be easily assessed.
  3. Food and medications - You can take your usual food and supplements unless otherwise specified by your healthcare professional. Fasting is not required in obstetrics USG or Level 2 scan.
  4. You need to drink plenty of water to make your bladder full just before the scan begins.
  5. Smoking/Tobacco- Avoid smoking or tobacco chewing before the test as nicotine in these substances may case vasoconstriction which may alter the test results especially in case of obstetrics color doppler.
  6. Reports- Carry your relevant medical and lab reports while going for USG level 2 scan. These may aid in better evaluation of your condition.
  7. Hygiene- Maintain hygiene and do not apply oils or creams over the area which need to be evaluated.
  8. Take a company- Get accompanied by a family member or a friend to have emotional and physical support.

What Is the Procedure for Usg Level 2 Ultrasound?

The procedure for Level 2 ultrasound is similar to routine pregnancy ultrasound and may involve the following steps:

  1. Before the scan begins you need to remove ornaments and clothes from the body to expose the Abdomino-pelvic region which is being examined.
  2. You will be then asked to lie on the examination table.
  3. Your Sonographer/Obstetrician will then apply gel/lubricant over the Abdomino-pelvic area for the smooth gliding of probe over it.
  4. Then a transducer/probe will be moved over the skin of Abdomino-pelvic region that sends high frequency sound waves to the maternal and fetal body parts. These sound waves are reflected back and are recorded by the ultrasound machine and converted into images or graphs. These images will be interpreted by your sonographer to assess your condition.
  5. At the end of scan, tissue paper will be provided to wipe off gel/lubricant from your belly surface.
  6. You will be allowed to leave once the scan is over.

What Happens After Level 2 Sonography?

  1. Level 2 Ultrasound test usually takes about 30-45 minutes for detailed evaluation. However, it may lasts longer in case of multifetal pregnancy or if there is any associated co-morbid conditions.
  2. You will be allowed to leave soon after the scan.
  3. After the procedure got over, your Sonographer will analyze the result and make a report of your existing conditions.
  4. You will get the reports on the same day.
  5. These reports will be evaluated by your Obstetrician:
    • To check normal fetal growth and development
    • To rule out fetal anomalies
    • To rule out any pregnancy related complications
    • To identify any abnormality and make a proper management    plan for your current pregnancy. 
  6. Level 2 ultrasonography is a safe procedure with no risks/side effects.
