Biological Indicators: What Are They?

Biological indicators (BIs), according to ANSI/AAMI and ISO, are test items containing live microorganisms that demonstrate a specific level of resistance to a particular sterilising process.

A biological indicator provides data on whether the conditions were satisfied for a sterilisasterilisation procedure to eliminate a certain number of germs, demonstrating pprocess'sconfidence. Endospores or bacterial spores are the microorganisms most usually used in BIs.

The choice of bacterial spores for a  method also considers the spores' known resistance to that method.

For instance, Geobacillus stearothermophilus spores are used in BIs that monitor these sterilising processes because they are very resistant to steam and vaporised hydrogen peroxide. While mechanical and chemical indicators do not ensure sterilisation, they do aid in identifying operational mistakes (such as overloaded steriliser or improper packaging) and equipment problems. With each steriliser load, mechanical and chemical monitoring should be performed.

Monitoring comprises confirming that the pressure, temperature, and exposure duration have reached the levels advised by the steriliser manufacturer and recording this information in your sterilisation records. This could be the first sign because these characteristics can be seen during the sterilisation cycle.

The procedure for testing biological indicators:

  • A carrier material on which bacterial spores with specific resistance to the sterilising procedure have been put makes up a biological indicator.
  • A glassine bag or a vial is used to contain the carrier substance.
  • To determine if any spores survived the sterilisation procedure, the BI is subjected to the process and then incubated under predetermined growth conditions.
  • If no spores develop, the test is successful.
