What is the BCR-ABL PCR QUANTITATIVE Test?

The BCR-ABL Gene Translocation test can aid doctors in the diagnosis and monitoring of chronic myeloid leukemia (CML) and acute lymphoblastic leukemia (ALL).

The BCR gene is usually found on chromosome 22 and the ABL gene is traditionally seen on chromosome 9. The BCR-ABL gene is hence formed, when parts of the BCR and ABL genes break off and switch locations.

This chromosome formed as a result of the switching and translocation is called Ph chromosome or Philadelphia Chromosome

Genetic molecular testing comprise of The BCR-ABL Gene Translocation (Qualitative and Quantitative) test can assist clinicians in treatment and it’s usually conducted in the form either of the two:

  1. Blood tests- Blood drawn from a vein
  2. Bone marrow aspiration or biopsy: Doctors may require the desired bone marrow sample by needle insertion into your hip bone

The entire procedure is carried out under anesthesia. If the test results display this gene, then you may have ALL or CML.

And it is necessary to seek professional therapy for further treatment planning.

Note: It is seen that most often than not, both the blood tests and bone marrow are evaluated as part of the initial diagnosis, however majority of the follow-up monitoring will be performed based on further blood samples.

Why is BCR-ABL Test Done?

This test can detect CML or Philadelphia chromosome (Ph)-positive ALL in patients and monitor their therapy. When signs and symptoms of leukemia are seen in the patient, initial testing is advised. The presenting symptoms of Leukemia are as follows:

  • Tiredness
  • Weight loss
  • Joint pain
  • Night sweats
  • Fever
  • Palpitations
  • Easy bruising
  • Platelet Loss
  • Anemia
  • Extreme confusion and seizures
  • Abnormal findings on a CBC

Uses of BCR- ABL Quantitative Test

PCR-based methods are used identify and quantify the tumor-specific BCR-ABL1 sequence that have been shown to be an ultrasensitive diagnostic/prognostic tool for this Philadelphia-positive leukemia.

That means- it can also detect the abnormalities even with less sample size (low level) and demonstrates high accuracy in the diagnosis process ahead.

Further testing methods

Furthermore, when the patient is not responding to the current treatment and treatment resistance is suspected or there is disease recurrence after remission, The BCR-ABL1 kinase domain mutation analysis might be performed further performed.

The fusion gene BCR-ABL1 encodes a protein, tyrosine kinase, which is an enzyme. This enzyme is responsible for the development of Chronic Myeloid Leukemia (CML) and Acute Lymphoblastic Lymphoma (ALL).

Yes, there is cure for CML if diagnosed in early phase. Treatment of CML slows down the progression of cancer, it may include:

  • Targeted therapy – tyrosine kinase inhibitor
  • Chemotherapy
  • Immunotherapy
  • Bone marrow transplant

Yes, the translocation and fusion of BCR-ABL1 gene results in unique transcripts, which are strictly produced by leukemia cells.
