A protein known as adenosine deaminase (ADA) activates white blood cells to help the body fight infection. Adenosine deaminase serum testing is used to evaluate the ADA. It helps with tuberculosis (TB) diagnosis, which increases ADA levels. With this test, it can also be determined if tuberculosis may have progressed to the lining of the lungs (pleura).

TB symptoms include coughing, chest pain, and breathing difficulties. The same symptoms are commonly seen in other disorders as well. As a result, this evaluation assists in making an accurate diagnosis and prompts a remedy. It may also be given to someone who has a compromised immune system, lives near to an infected person, or is more likely to get TB.

AbstractsBackground

The enzyme adenosine deaminase (ADA) can help diagnose tuberculous pleural effusion (TPE). The purpose of this study is to identify the variables influencing pleural fluid ADA levels and to establish the ideal ADA levels for TPE diagnosis for various age groups.

Methods

The study's time period included the period from January 2007 to October 2011. A total of 156 individuals who underwent pleural fluid ADA for the purpose of pleural effusion investigation were examined. Demographics, pleural fluid properties, and peripheral blood counts were among the variables investigated.

Results

In comparison to non-TPE patients (30 37 IU/L), the mean pleural fluid ADA was significantly greater in the TPE group (100 35 IU/L). Pleural fluid ADA, age, pleural fluid protein, LDH, and fluid absolute lymphocyte count all showed a strong association. Age was associated with the strongest connection (r = 0.621). The ROC for ADA has an area under curve (AUC) of 0.887 for patients under the age of 55. The diagnosis of TPE was made with a pleural fluid ADA of 72 IU/L, which had a 95.1% sensitivity, 87.5% specificity, 95.1% positive predictive value (PPV), and 87.5% negative predictive value (NPV). The AUC is 0.959 for patients over the age of 55. PPV was 62%, specificity was 80.4%, and sensitivity was 94.7% for ADA at 26 IU/L.

Conclusions

Pleural fluid ADA has a very strong negative connection with ageing. A lower ADA cut-off should be utilised to rule out TPE in older patients.
