What is Acetylcholinesterase Test

The red blood cell enzyme acetylcholinesterase, often known as AChE, is responsible for catalysing the breakdown of choline esters, which predominantly serve as neurotransmitters. By hydrolyzing the neurotransmitter acetylcholine, the enzyme drastically reduces the transmission at cholinergic synapses. To obtain the level of enzyme present in the cells, a medical treatment is recommended. Malnutrition and chronic liver disease are two conditions that might result in decreased cholinesterase levels. Additionally, during pregnancy, renal conditions, trauma, and some malignancies, the enzyme activity declines. Significantly reduced levels of cholinesterase activity typically signify overabsorption. The chemicals are often absorbed in the skin, lungs, and gastrointestinal tract before becoming toxic.

Uses

This procedure is helpful in figuring out how harmful organophosphates are. The test is primarily focused on those who frequently come into contact with organophosphate substances, such as individuals who work in the chemical and agricultural industries. It can also be used to evaluate whether someone is more likely to be exposed to certain substances.

The test may also be done in advance of surgery to determine whether succinylcholine, a muscle relaxant used for anaesthesia, has a history of causing reactions or whether there is a family history of postoperative paralysis.

Organophosphate poisoning

Organophosphate poisoning can result from intentional consumption, occupational or unintentional exposure, or nerve gas chemical warfare.

Due to variations in dose, agent toxicity, and mode of administration, presentation might vary greatly.

The diagnosis is typically based on a history of exposure and includes telltale symptoms of cholinergic excess, but it can be challenging if the patient has been exposed unintentionally or is asleep or disoriented.

Reanimation, supportive care, decontamination, and the administration of atropine are the normative treatments.

Only organophosphates that block acetylcholinesterase are referred to as "organophosphate toxicity." The focus of this discussion is pesticide toxicity.
