The 1,3-d-glucan (BDG) test concept is appealing since it offers a noninvasive test approach to identify invasive fungal diseases (IFIs). IFIs have increased in frequency as more people undergo protracted immunocompromised periods.

What is the purpose of this test?

  • This technique is used to quantify the presence of (1-3)-Beta-D-Glucan (BDG), a marker for invasive fungal infection, in serum samples. It can be used for prognostic as well as diagnostic purposes.
  • Most fungi's cell wall is composed primarily and specifically of a polymer called 1,3-d-glucan (BDG).
  • As a biomarker for conditions like invasive aspergillosis (IA) and invasive candidiasis, BDG can be found in the serum during invasive fungal infections (IFI).

How do beta-glucans work?

  • Beta-glucans are soluble fibres in the cell walls of various plants, fungi, and bacteria.
  • The body may not be able to absorb cholesterol from the diet when beta-glucans are present.
  • Enhancing the molecules that fight infections may help boost the immune system.

Ill-Effects of beta-glucans on the body:

When administered orally: Foods contain beta-glucans frequently. For up to 8 to 12 weeks, using higher doses of beta-glucans as medicine may be safe. There are no known adverse consequences from ingesting beta-glucans. Beta-glucans may be safe when used topically for a brief length of time. Some people may develop skin rashes from them.

Applications:

  • Glucan testing may be employed as an adjunct diagnosis, start prescription or surgery, and improve overall clinical surveillance in patients with invasive fungal infections, including endophthalmitis, who have been diagnosed with such conditions or have a clinical suspicion of having them.
  • To properly evaluate the value of BDG testing in patients with fungal endophthalmitis, more clinical research is required.

Why is this examination suggested?

  • The Fungitell test is recommended with other diagnostic techniques for the presumptive diagnosis of a fungal infection.
  • Certain fungi, such as Cryptococcus, which produce very little (1,3)-beta-D-glucan, are not detected by this test.
  • Moreover, the Blastomyces dermatitis yeast phase has minimal (1,3)-beta-D-glucan and might not be picked up by the assay.
