What is NPM1 Gene Mutation Test?

The NPM1 gene codes for a protein called the nucleophosmin which is found in the nucleolus, a small region inside the nucleus of the cell. Nucleophosmin circulates between the nucleus and the fluid surrounding it (the cytoplasm). It is thought to be involved in many cellular functions, including protein formation, DNA replication, and the progression of the cell through the step-by-step process of replicating itself (called the cell cycle). Nucleophosmin binds to another protein called ARF in the nucleolus, keeping it in place and preventing it from being broken down. Because it is involved in pathways that prevent cells from growing and dividing, the ARF protein is considered a tumour suppressor.

Why it is done?

One of the most common recurring genetic lesions in acute myeloid leukaemia is the NPM1 (nucleophosmin) mutation (AML). This AML type is characterised by myelomonocytic or monocytic features and typically manifests in older adults with a normal karyotype. Prevalence rises with age, with 2% to 8% of childhood AML and 27% to 35% of adult AML affected. The most common NPM1 mutation is an insertion at nucleotide position 866-870 (exon 11), which accounts for 90% to 95% of all NPM1 mutations. In the absence of FLT3-ITD, NPM1 mutations identify a prognostically favourable subgroup.
