What is Metachromatic leukodystrophy Test?

Metachromatic leukodystrophy is an inherited disorder characterised by sulfatide accumulation in cells. This accumulation has a particular impact on nervous system cells that produce myelin, the substance that insulates and protects nerves. White matter is a tissue made up of nerve cells covered in myelin. Sulfatide accumulation in myelin-producing cells leads to progressive destruction of white matter (leukodystrophy) throughout the nervous system, including the brain and spinal cord (the central nervous system), as well as the nerves that connect the brain and spinal cord to muscles and sensory cells that detect sensations such as touch, pain, heat, and sound (the peripheral nervous system).

Why it is done?

Most people with metachromatic leukodystrophy have mutations in the ARSA gene, which codes for the enzyme arylsulfatase A. This enzyme is found in cellular structures known as lysosomes, which serve as recycling hubs for the cell. Arylsulfatase A is a lysosomal enzyme that aids in the breakdown of sulfatides.

What is the procedure for Metachromatic leukodystrophy Test?

To look for signs of metachromatic leukodystrophy, your doctor will perform a physical examination, including a neurological exam, as well as a review of your symptoms and medical history.

To diagnose the disorder, your doctor may order tests. These tests also aid in determining the severity of the disorder.

  1. Laboratory tests Blood tests are performed to detect an enzyme deficiency that causes metachromatic leukodystrophy. Sulfatide levels in urine can be measured.
  2. Genetic examinations. Your doctor may run genetic tests to look for mutations in the gene linked to metachromatic leukodystrophy. He or she may also advise testing family members, particularly pregnant women (prenatal testing), for gene mutations.
  3. Nerve conduction analysis A small current is passed through electrodes on the skin to measure electrical nerve impulses and function in muscles and nerves. This test may be used by your doctor to detect nerve damage (peripheral neuropathy), which is common in people with metachromatic leukodystrophy.
