What is the D-DIMER Test?

The D-Dimer test (fibrin degradation fragment Test) looks for D-Dimer, a protein fragment formed when a blood clot dissolves. This test is used to diagnose coagulation problems, pulmonary embolism, and DVT. The test is commonly performed when someone has trouble breathing or chest pain. 

If your D-Dimer levels are normal, you don't have a clotting disorder. If the levels are higher than usual, you should see your doctor to rule out clotting disorders or other medical conditions.

Other names of D-Dimer test include: Fragment D-dimer, or fibrin degradation fragment

What Is the Use of D-Dimer Test

D-dimer test is used to detect a blood clotting disorder. These disorders include the following:

  1. Deep Vein Thrombosis (DVT): Blood clot that's deep inside the vein. 
    These generally affect the lower limbs; however, they can also occur in different parts of the body.
     
  2. Pulmonary Embolism (PE): Is a blockage that occurs in an artery that is  the lungs. It generally occurs incase a blood clot in another part of body breaks loose, also travels to directly the lungs. 

It is important to note that the DVT clots are also a common cause of PE in general.

Disseminated intravascular coagulation (DIC): Is a condition that causes many blood clots to form together. They can also form throughout the body, this causes organ damage eg. By traumatic injuries and certain types of infections, also by cancer.

Why Is D-Dimer Test Required?

Symptoms of DVT and PE are the indications for which the test should be carried out. Some of the symptoms are listed as follows:

For DVT:

  1. Tenderness or leg pain experienced by the patient
  2. Leg Inflammation
  3. Red Streaks or Redness seen on the legs of the patient

For PV

  1. Troubled breathing
  2. Cough 
  3. Frequent or acute chest pain experienced by the individual
  4. Increased or Rapid Heart beat

Why Done?

The D-dimer test is used to determine the presence of a blood clot that isn't healthy (thrombus). The fragment d-dimer test is used to help rule out the following conditions:

  1. Thrombosis of a deep vein (DVT)
  2. A pulmonary embolism is a type of blood clot that occurs in the lungs (PE)
  3. Stroke

This test may also be conducted if a person exhibits pulmonary embolism symptoms such as:

  1. Fast heart rate
  2. Forced breathing, sudden shortness of breath
  3. Hemoptysis (coughing) (blood present in sputum)
  4. Chest pain caused by lung disease.

Guidelines To Follow Before, During and After the Test

Here are some guidelines an individual can refer to once they have decided to undergo D-Dimer test

The required documents needed for this test includes your valid government ID and an appropriate Doctor’s prescription.

Before the Test

 It would be better if the patient can fast overnight.

During the test:

A little sting may be felt when the needle is inserted or taken out. This normally would take less than the scheduled five minutes.

After the test:

  1. The patient should keep the dressing strip on for another 15 min.
  2. However, No other or specific type of care is required ahead.

Results of the Test

If the results are either low or normal D-dimer levels in the blood, this indicates that means the individual does not suffer from any clotting disorder

If the levels are higher than normal levels of D-dimer, it may indicate that the individual has a clotting disorder. However, it does not show either the location of the clot or the type of clotting disorder

Also, other conditions that can also mimic higher levels cause high D-dimer levels eg. Pregnancy, heart disease, or recent surgery

A normal range is usually between 0-0.50 milligrams per liter (mg/L) of blood

Other tests that may be used to detect D-Dimer is the Doppler ultrasound, CT angiography and Ventilation-perfusion scan.

You'll have blood extracted from an arm vein. 

How Does One Prepare for the Test?

A brief clinical history should be given. It's also a good idea to fast overnight.

  2. A valid government ID.

After Care

Leave the dressing strip on for 15 minutes. No particular type of care is required.
