The Tuberculosis (TB) is known to be a contagious infection that is known to usually attack lungs of the individual. It has spread to different parts of your body, like your brain and spine. This type of bacteria is called Mycobacterium tuberculosis and to understand what causes it.
The Active Form: The Tuberculosis (TB) disease could occur in the various anatomical sites, but this will be divided into the two main groups, such as the pulmonary TB disease and any extra pulmonary disease. Because the TB disease predominantly affects the lungs, this form is also commonly referred to as your pulmonary disease.
Diagnostic Testing Procedures for TB
1. Bacterial culture
Your MTB is slow growing in nature. Due to any challenges of the cultivation, use of just one liquid and one of the solid media is recommended further to maximize the recovery process.
2. Advanced testing methods
The most widely used US FDA-cleared automated systems for any rapid detection of the mycobacteria using the liquid media would include: BacT/ALERT 3D (Biomerieux), the BACTEC MGIT (Becton Dickinson) and the VersaTREK (Thermo Fisher).
These employ automated continuous monitoring of carbon dioxide (CO2) production or oxygen (O2) consumption to detect MTB.
3. Nucleic Acid Tests in TB
Your Xpert MTB/RIF is known to utilize the nested real-time PCR for any qualitative detection of MTB complex and the RIF resistance. The diagnostic algorithm using NAAT serves as an initial diagnostic test in which the pending culturing is shown.
The Rapid turn-around time for the NAAT test and AFB test is necessary for the diagnosis and your prompt initiation of your therapy pending culture results

Conclusions of these Advanced Tests
Common target genes for TB by NAAT , the Various genes are targeted by NAAT.
Apart from that, there are 2 more tests for tor TB. Such as: There interferon gamma release assays (IGRAs) that have been approved by FDA and are available. for TB screening. The tests listed are as follows:
- QuantiFERON TB Gold In-Tube test (QFT-GIT)
- T-SPOT TB test (T-Spot)

What is the requirement of these tests?
If you experience the following TB symptoms, make sure to get yourself tested for TB using the advanced testing procedures as mentioned above.
- Fatigue
- Chest Pain
- Night Sweats
- Blood Cough
- Chills
- Cough lasting > 2 weeks
Note: Tb has the ability to affect not just the lungs, but other parts of the body as well. E.g. Brain, spine etc
Risk Factors
Have HIV or another disease that weakens your immune system
- The illicit use of illegal drugs
- The higher rate of your TB infection in enclosed spaces, example- prison or the nursing home
- Any specific medicines would suppress the immune system
Tests Results Interpretation
- Inactive (latent) TB-This means TB bacteria are present in your body but are not active and are not causing symptoms. You are not contagious, although it's possible for you to develop TB in the future.
- Active TB or TB disease- This means TB bacteria are active in your body, and you are contagious.
Guidelines to follow before, during and after the TB Testing
Here are some guidelines an individual can refer to once they have decided to undergo TB Tests in Ganesh Diagnostic and Imaging Center
The required documents needed for this test includes your valid government ID and an appropriate Doctor’s prescription.
A. Before the Test
The preparation for multiple tests for TB testing may require some preparation ahead.
Usually, Fasting is not required before the test. But, it is recommended to consult your doctor before undergoing advanced TB Tests
B. During the Test
During the blood test, the doctor would take the blood sample from the vein in your arm, using the small needle.
Then, a small amount of blood would be drawn and collected into the corresponding test tube or the vial.
You might feel a little sting when the needle pricks or while it is being removed. This would usually take less than even five minutes.
C. After the Test
There would be slight tenderness at the injection site without any major concern. This patient is expected to get back to their daily life immediately.

Are there any risks to this test?
There could be risks seen that are as follows:
- Bleeding
- Infection
- Bruising
- Feeling Lightheaded
How is tuberculosis treated?
Your TB infection and this disease is said to usually be treated with the following drugs:
- Isoniazid (Hyzyd®).
- Revampingz (Rifadin®).
- Ethambutol (Myambutol®).
- Pyrazinamide (Zinamide®).
- Rifapentine (Priftin®).
Note: You are recommended to take all the medication that the healthcare professional would prescribe, or not all of your bacteria would be destroyed
You should consume these medications for as long as advised from 9 to 12 months.
1st line of treatment: Out Of these approved drugs, the ones mentioned above, such as: isoniazid (INH), rifampin (RIF), ethambutol (EMB), and pyrazinamide (PZA) are considered to be the first-line anti-TB drugs and thus, they form the core of this standard treatment regimens
While, Rifabutin (RBT) and rifapentine (RPT) might be considered first- line drugs under some certain circumstances.
Directly Observed Therapy (DOT)
It is usually seen that the DOT treatment is one of the most preferred core management strategy would be recommended by the CDC for your treatment of the TB disease and at times also for latent tuberculosis infection (LTBI) treatment.
Nearly, all treatment regimens that are drug-susceptible TB disease would be given intermittently if they are directly observed.
Note: The Drug-resistant TB disease should be considered as a daily regimen and under the direct observation.
There are also no intermittent regimens for the treatment of the multidrug-resistant (MDR) TB.
Fixed-Dose Combination Drugs
The use of this fixed-dose combination capsules or the tablets facilitates this DOT administration by minimizing chance for any error through the even use of fewer tablets and this might reduce the risk of any acquired/ Multidrug resistance (MDR) since one of the medication that cannot be selectively taken.
Self-Administered Therapy
Patients on their self-administered therapy should be monitored routinely for Patient Adherence and further follow-up visits.

Frequently Asked Questions (FAQs)
What sample is used for CB NAAT test?
The Cartridge Based Nucleic Acid Amplification Test (CBNAAT) is known to be a rapid molecular diagnostic test. It is mostly used in the diagnosis of your Tuberculosis (TB) and any Rif-resistant Tuberculosis (RR-TB) in your NTEP. The results are mostly obtained from any unprocessed sputum samples which is in about 2hours and this helps in the early detection and treatment of any TB patients.
What is the effectiveness of CBNAAT in the diagnosis of extra pulmonary tuberculosis?
The Cartridge-based nucleic acid amplification test (CBNAAT) is considered to be a very useful assay for any early diagnosis of the extra pulmonary cases. This would accurately identify the false negative samples by the smear microscopy.
How does the Genexpert work in testing for TB?
The Genexpert diagnoses TB by detecting the presence of TB bacteria. The test is a molecular TB test which it detects any DNA in the TB bacteria. It uses the sputum sample and this would show the results in less than a time period of 2 hours.
How sensitive is GeneXpert in urine?
The GeneXpert MTB/RIF, the real-time PCR, is a sensitive and a specific test with around 131 cfu/mL limit of any detection that also allows for any early detection of the TB disease. The AFBs are known to intermittently shed in your urine and your sensitivity of the GeneXpert . This could be improved by using the early morning urine for any extended periods ranging from around 3 to 5 days.
What happens if GeneXpert is negative?
There is an enhanced accuracy and it has a negative predictive value of the GeneXpert, which was found to be extra better than the AFB staining. Thus, the negative GeneXpert test would be able to rule out your TB. Further, the negative GeneXpert bears a positive smear microscopy, and the results indicate any presence of the NTM.
