An intravenous pyelogram (PIE-uh-low-gram), also known as an excretory urogram, is a urinary tract X-ray. Your doctor can see your kidneys, bladder, and the tubes that carry urine from your kidneys to your bladder during an intravenous pyelogram (ureters).

An intravenous pyelogram is a test that looks at your kidneys, ureters, and bladder. It allows your doctor to assess the size and shape of these structures as well as their functionality.

If you have signs and symptoms of a urinary tract disorder, such as pain in your side or back or blood in your urine, your doctor may recommend an intravenous pyelogram.

An intravenous pyelogram may be used to help diagnose urinary tract conditions such as:

  • Kidney stones
  • Bladder stones
  • Enlarged prostate
  • Kidney cysts
  • Urinary tract tumours
  • Structural kidney disorders, such as medullary sponge kidney — a birth defect of the tiny tubes inside the kidneys

Inform your doctor prior to an intravenous pyelogram if you:

  • Do you have any allergies, especially to iodine?
  • Are you pregnant or suspect you may be pregnant?
  • Have previously had a severe reaction to X-ray dyes

Before an intravenous pyelogram, you may need to refrain from eating and drinking for a period of time. Your doctor may also advise you to take a laxative the night before the exam.

You lie on your back on an exam table for an intravenous pyelogram. The X-ray machine is usually attached to or built into the table. An X-ray image intensifier — the machine component that produces the images — is placed over your abdomen. After you've settled into a comfortable position on the table, the exam proceeds as follows:

Before any dye is injected, X-rays of your urinary tract are taken.

  • An X-ray dye is injected into your IV line.
  • As the dye flows through your kidneys, ureters, and bladder, X-ray images are taken at regular intervals.
  • You may be asked to urinate again near the end of the exam.
  • You then return to the exam table to allow the medical team to obtain X-ray images of your empty bladder.

When your intravenous pyelogram is finished, the IV line in your arm is removed and you can resume your normal activities.
