What is an X-ray Hysterosalpingography (HSG) Test? 

A hysterosalpingogram (HSG) is known to be a fluoroscopic treatment, to diagnose issues related to fertility. This test uses the X-ray images to capture the uterine walls - to obtain an image of the uterus and fallopian tubes.

A dye is injected into the patient’s body that can further produce a clearer picture in this procedure. 

Why is the HSG scan done? 

This technique is most frequently used to investigate infertility or repeated spontaneous abortions.

Furthermore, it is used for the following:

  1. To check the success of a tubal ligation/tubal reversal
  2. To plan for further imaging

Guidelines to follow before, during and after the test

Here are some guidelines an individual can refer to once they have decided to undergo Hysterosalpingography (HSG)

The required documents needed for this test includes your valid government ID and an appropriate Doctor’s prescription.

A. Before the test

  1. Nothing Specific to prepare for the HSG Test in advance.
  2. Typically, discuss with the HCP if you are pregnant
  3. Inform the HCP if you are allergic to the dyes, that would be injected into the body 

Note: You might need to take a pain medicine- an hour prior to the procedure

  • Also, take antibiotics beforehand itself, to prevent an infection.

 B. During the Test

HSG Test Procedure:

The HSG is done in the hospital or clinic or diagnostic center. It is best to have the HSG within first half of menstrual cycle (days 1 to 14) itself.

  1. You have to lie on the back and your feet should be placed as it is a pelvic exam. The device is called – as a speculum will be inserted into the patient’s vagina. It holds against the walls of vagina apart to allow the cervix that can be viewed. The cervix is then cleaned. 
  1. The end of the patient’s cervix might be further injected with the local anesthesia (for pain relief). You might also experience a slight pinch or even tug. 
  1. There are 2 methods of inserting the dye. During one method, the cervix will be grasped with a device, so that it can be held steady. 
  1. An instrument is further called a cannula, which is then inserted into the cervix. 
  1. On the other hand, in the second method, a thin plastic tube would be passed into the cervical opening. The tube usually has a small balloon towards the end which is inflated. This balloon is used to keep the tube in place within the uterus. 
  1. The speculum is then removed, and you will be then placed beneath an X-ray machine. The fluid is subsequently placed through the use of cannula or the tube into the patient’s uterus and the fallopian tubes. 
  1. This fluid may later on also cause cramping. In case, the tubes are blocked, the fluid would cause them to further stretch. 
  1. The X-ray images are then taken as the fluid would fill the uterus and the tubes. You might also be asked to change your position. In case of no blockage, the fluid would spill slowly from the far ends of the tubes. After it has spilled out, the fluid would absorbed by the body. 
  1. After the images have been taken, the speculum and cannula or tube would be removed.

C. After the test

You may experience the following side effects after the test. These include the following:

  1. Cramps
  2. Dizziness
  3. Nausea and indigestion
  4. Slight vaginal bleeding for a day or two

Normal HSG Test Report

The normal HSG report would indicate no blockage in your fallopian tubes and thus no abnormality would be visualized in the uterus.

 In case your fallopian tubes have been blocked, the test reports would come as abnormal.

 However, a normal HSG report will also not necessarily indicate that the patient’s fertility is also normal.

It just indicates that uterus and both the fallopian tubes are normal and not blocked.
